Family Law Mediation Consultation Request Form

To request a family law mediation consultation, call us at (916) 419-1160 or complete the online form below. You and the other party will be emailed a Mediation Questionnaire to complete after we receive your submission. Before you submit the mediation consultation request, you must speak with the other party to ensure he/she is agreeable to a mediation consultation. The first-scheduled family law mediation consultation is a free 30-minute appointment that does not include document review; additional-scheduled/rescheduled family law mediation consultations are charged at the normal hourly rate. Submitting this form does not create an attorney/client or mediator/client relationship and does not guarantee service. You must complete all information to receive a response. If you select an option that says "I'll Call GFL," do not submit the form since you must call in to our office to schedule your consultation. No solicitations.
